Alimentary tract perforations represent an emergency and life-threatening condition requiring prompt diagnosis and surgical treatment in most cases. Diagnosis depends on clinical suspicion: The clinical symptoms of free perforation are associated with the underlying cause of the perforation, and the clinical presentations of patients with a perforated viscus are quite variable. Imaging examinations play an important role in order to determine the correct diagnosis and ensure an appropriate treatment to the patient. The purpose of this book is to illustrate and discuss causes and imaging features related to cases of alimentary tract perforation from the esophagus to the rectum, occurring in radiology practice. Signs of a pneumoperitoneum on plain abdominal fi lm, on abdominal ultrasonography, and on Multi-detector row Computed Tomography are extensively illustrated; moreover, the role of plain abdominal fi lm, abdominal ultrasonography, and Multi-detector row Computed Tomography are addressed. In addition, imaging of gastrointestinal tract perforation in the pediatric patient, in the elderly patient, and in the oncological patient are presented. We hope that this book will provide important information to Residents in Radiology, Radiologists, and Physicians daily involved in the Emergency Department in the management of patients with suspected alimentary tract perforation.

The 21st InternationalConference on Information Processing in Medical Imaging (IPMI) was held July 5–10, 2009 at the College ofWilliam and Mary inWilliamsburg, Virginia, USA. The conference was the latest in a series of biennial scientific meetings, the last being held in July of 2007 in the south of The Netherlands, during which new developments in the acquisition, analysis, and use of medical images were presented. IPMI is one of the longest running conferences devoted to these topics in medical imaging. The first IPMI conference was held in 1969, when a group of young scientists working in nuclear medicine gathered to discuss the current problems in their field. Since that time the conference has expanded into other medical imaging acquisition modalities, including ultrasound, optics, magnetic resonance, and x-ray imaging techniques. IPMI is now widely recognized as one of the most exciting and influential meetings in medical imaging, with a unique emphasis on active participation from all attendees and a strong commitment to vigorous discussion and open debate.

The past 30 years bear witness to a profound evolution in our ability to diagnose pediatric cancers. What were once common diagnostic dilemmas are now routinely categorized with the help of ancillary tests such as immunohistochemistry, fl uorescence in situ hybridization, and a myriad of molecular diagnostics tools. We are now moving into an unprecedented new realm wherein personalized medicine will require panels of tests not only for diagnosis but also for customizable therapy guided by somatic mutation analysis. As our ability to diagnose pediatric cancer has steadily grown, our diagnostic specimens have shrunk. Previous surgical approaches often required complete and sometimes radical tumor excision prior to embarking on therapy. Now, however, we initially obtain small biopsies for tumors that are treated with neoadjuvant therapy and subsequently excised.

As the concept evolved, and with the close interaction of my rheumatology colleagues, we decided to embark on a different approach and develop the book with input from both radiologists and rheumatologists, as neither exists separately in real life, so as to serve the needs of all.
